Walk-Ins
When entering Sportsworld you will be required to sign in to our
session guest registry. This to be done by all people entering Sportsworld
regardless if you are skating or not.

Booked Functions
Option 1
List all of your guests on this form and sign it. It is a guest list and Concession
order form. You do not require individual parent signatures of your
guests. Each form can hold up to 20 people, if you require more name slots
please print off an additional form.
Option 2
Each form can hold up to 50 people, if you require more name slots please print
off an additional form. You do not require individual parent signatures of your
guests. The person or organization coordinator hosting the party can print & sign. This form is valid for the date listed on the sheet only. When you host another party or group activity you must fill this form out again.
Option 3
If your booking is between Mon-Fri 9am-5pm. Each form can hold up to 50 people, If you require more name slots please print off an additional form. You do not require individual parent signatures of your guests. The person or organization coordinator hosting the party can print & sign. This form is valid for the date listed on the sheet only. When you host another party or group activity you must fill this form out again. *If your booking is any other private slot you and your guests will have your own session guest list registry to sign in as you enter. You do not need to print off this form.
All booked options must have their form completed if it is required before they
come in. If not, you will have delays in checking in your party which may effect
your table reservation time if applicable. DO NOT fax or mail this form just bring
it in completed. Having the form completed will help speed up entry at the door.
